If I'm not mistaken, it looks like the TNT Carts are colliding inside the cannon. If this is the case, there is no water inside of the canon therefore the TNT Carts' explosions are not being cushioned by water.

I believe Minecraft Realms only supports survival at the moment, although I may be wrong.

Have you been able to replicate this multiple times? If so, how are you replicating it, I'd like to test it myself.

This seems expected. It sounds as if you are hitting the Enderman into the water thereby teleporting him. However, he also takes the damage from the water and that kills him.
