Just had this issue on 1.13 on Foxy No-Tail's Patreon Realm. Two patrols spawned back to back, just strutting right through the end. I was the only player online at the time.
Edit: Immediately after leaving this comment, another patrol spawned. Still the only one online. Also maybe worth noting: I don't see any Endermen in view distance. I do have a small bit of water and a full beacon set up using emerald blocks. Perhaps one of these items is being registered as a valid spawn location for them despite it being in the End?
Spent the entire evening trying to mine out a chunk and this problem was occurring the whole time with most blocks needing mined 3-4 times to actually break. My Unbreaking III Diamond Pickaxe was worn almost all the way down after clearing 7 layers and it took 3-4 times as long to do it. I might as well just make a bunch of iron picks and dismantle the beacon.
This bug is nearly game-breaking and is most certainly not resolved in current version.
The only time I encounter this issue is when my my internet connection is very poor or there is a lot of network traffic. You are likely being disconnected because your connection is timing out while trying to load all of the chunk data for the Nether.

Owners can create a backup at any given time by downloading the world. Any time you download the world, it also uses that download as one of the backups server side..
