On realm using diamond pickaxe with efficiency 2, often when rapidly digging in netherrack the blocks reappear and have to be mined again.
With haste 2 beacon and efficiency 5 pick this is happens for stone, etc also.
edit: possible workaround
I found that if I don't have any unbreaking on the tool I don't get block lag. I can sprint insta-mine netherrack with an Efficiency V pickaxe without a beacon. If I use my Unbreaking III / Efficiency V pick I get block lag all the time.

Can confirm for 1.12, on windows 10, xbox one and mobile. its mostly a realms issue from what i have seen.

I've experienced this with just Efficiency 3+ pickaxes, also with beacons & conduits. Combined with water affinity, I had an horrendous time taking down an Ocean Monument. Blocks re-appearing up to 4 times.
This happens all the time on a local multiplayer world. Even when placing blocks, they sometimes disappear and reappear a second later, causing a lot of falling when towering up. Mining through Netherrack in the Nether when searching for Ancient Debris and mining ores with fortune is the worst of all. Because of all the things happening at once (unbreaking, mending, picking up blocks/ores) blocks sometimes need to be mined like 3-5 times. I hope the developers know this issue isn't resolved by a long shot.

We've had comments from 5 of you since then. Two confirm that the issue is fixed for them, and the other 3 report that it's still happening. We are waiting to see if more comments come in before we decide whether to reopen this ticket. If there are many more such comments, it probably means Mojang missed a possible cause and we'll ask whether they want us to reopen this ticket or start a new one.
@unknown and @unknown: You haven't contributed any information about your particular experience of this bug in prior comments. It would be helpful if you would do so now. Please briefly describe the issue as it affects you, focusing particularly on any way it differs from the original report.
@unknown: It's not at all clear whether you're talking about the same issue. At the very least, you're conflating multiple issues, because blocks reappearing after mining is the opposite of blocks disappearing after placement, and those are likely unrelated problems. Placed blocks disappearing in a local multiplayer (but not a single player) world is usually caused by a network problem between the client and server, such as interference from another nearby wi-fi network operating over the same channels. That needs to be fixed in your router or other network device; it's not a bug and Mojang can't fix it. It could also be caused by a bug, but any network issue needs to be eliminated first, and in any case it would be a different bug than this one. Your issues mining Netherrack sound like they could be this issue, but as with the other two commenters, we haven't had any information from you describing your experience.
Platform: single player Windows 10 PC running Bedrock edition 1.17.40 on the local machine. The world was originally created in 1.16, if that matters
Situation: mining a large area in the overworld with a haste 2 beacon and netherite Efficiency V, Unbreaking III, Silk Touch, Mending pickaxe. This results in "instant" mining of stone etc.
Observation: occasionally a block of stone will disappear momentarily then immediately reappear when mining
Mr. K: since this is an older report and we know Mojang has made changes to fix it, could you create new report and include device details and a video showing what it looks like for you now, and mention this one?
Another user is reporting block lag specifically in the nether (MCPE-148201).
I'm having this issue with haste II and efficiency V as well since 1.12
