It is in adventure mode so dispensers wouldn't work and it is supposed to be a bit of a high risk high reward sort of deal where it is hard and you take damage from doing it but you get invisibility, jump boost, etc.
This also happens to me when I load and unload chunks in the overworld. This is at least happening on android
You should really offer a screenshot to confirm that this actually happened
I play on mobile and this happens 100% of the time when using touch controls. It also happens in singleplayer only worlds that have never been multiplayer
