I did it on Hypixel just now and it had a player head in the inventory, but it didn't happen.
Well this was on Mineplex, but I have not tried it on different servers yet.
Its not a note 5. Its a Galaxy S6.
The connection was perfectly fine.
Try and upgrade to Windows 7 or 8.
There wasn't a crash afterwards.
Just so you know, I figured out what was going on: there were too many installed things on the computer.
This has happened to me just now. It said entity.MinecartRideable.name for me too.
