I have found that turning v-sync off AND running the game in windowed mode solves the problem completely with no change to the max framerate setting.
This doesn't work for everyone though.
Turning vsync off does nothing for me, but adjusting the max framerate does. When I set it to 70 I get 50-60 fps with no input lag. If it is set higher than 70 or set to 0 then there is input lag at 60 fps. If it is set to 60 then I get no input lag at 30 fps.

I just tested this and a for me it only points the wrong direction if it's in your mainhand while in a menu as well as sometimes when it is picked up in a menu.
It's still a problem.

4: pick up items

I have the same problem, whenever lightning strikes, no matter how far away the bolt is, it plays an explosion sound as if it struck near the player. So what he meant is that it sounds like the lightning is right next to the player when it is not.

For Windows 10, running the game in windowed mode solves the problem for me.
