This isn't a duplicate issue, I can use bonemeal in a dispenser on grass that is stationary, But when it gets actuated by a piston and the dispenser is activated while the grass is moving it creates a hard crash for the server.

There's currently 2% of chance for a Village to be generated as Zombies Villagers.
If your village got some cobwebs and look in ruins, and some zombies spawned in it instead of real Villagers, you probably found one.
Please be sure to verify this possibility.

I got the same problem that Colin McGee mentionned with elytras (on server jar version, my friends got the issue too). It seems to occurs each time you're changing dimension.
This isn't a bug, it might be a suggestion (evolution).

Also you can't open the ESC menu, you're stucked on the chat interface. On server the only way to escape the bed when it happens is to kick the player. On Solo, you're forced to restart the game.
Confirmed on client and server side.
Sometime, reload all th chunks with the F3 + Q shortcut helps in Solo Mode.

As Brad Corbin said, the little end gateway warps you to the starting platform, and if you fly out or construct a bridge to reach the outer islands, no end cities generate.
Same problem on 18w50a 19w02a and 19w03b for me. There's no End Cities in my End, also trying to locate them with the /locate command bring you in the middle of the void
It happened to me on the server snapshot 18w50a version.
I didn't have the problem on a solo world yet.
Strangely it seems to work fine with chests in End Cities.
But also doesn't work with custom NBT structures (e.g. through datapacks) containing chests with loot table since 20w22a.
