In my case, I created and played the world in 15w41b. Going to the Nether (placing an obsidian portal frame with 10 block, the minimal size in the middle of a lake on top of two dirt blocks) and fell a great distance upon spawning into the newly generated Nether (1 Health left from fall damage, full iron armor).
Looking around, the Nether Portal was nowhere to be seen, even in higher places (probably spawned a great distance away from it). Making an additional obsidian portal from within the Nether to the Overworld I spawned in a new portal frame (inside the frame this time, so as expected), but died because of fall damage (even though I didn't fall at all). It's also worth noting that this new portal was placed underground ❓ .
