I've had all these issues as well

Confirmed and also really annoying. Preventing this map I'm trying to make from working.

I also got this problem but couldn't kill the skeleton at all. I got around it by changing to peaceful mode and back.

This happened to me as well, I actually saw the flesh in the hand of the zombie as it was lying down just before it poofed into smoke.
