It seems to happen after you've come through a nether portal
We had this issue on my new 1.13 Server today.
It didn't happen to all doors, but to some, but they weren't affected before and now they don't have the issue anymore.
It seems to be only for a few minutes or something, then it will be back to normal
Can confirm for shulker_box, <color>_shulker_box does seem to work as expected.

It is, MC-26356 is Resolved AS "Won't Fix", which means the problem still persists in the game.
The place you are trying to summon an entity needs to be loaded.
This means you need to be near it, or it needs to be in the spawn chunks.
Probably intended because banners on shields are less wide than banners
Are the seeds flowing over farmland?
If so, they won't enter the hoppers because farmland is lower than one block.
They get stuck on the edge of the hopper.
It means that this bug/feature was intended by the developers

Well, "detailed walkthrough" is singular, so it is right. To avoid confusion, a comma could be added after "details"

I can't get it to work with any block, but that is probably just me

It sounds like that is just some intended behaviour of the spawn chunks.
When you are not an OP on a server, you can't break blocks or do anything around some of the chunks around spawn.
