Still occurs in 1.16.0.2 release upon destroying any block in any game mode.
In 1.16.0.2 release I can't be launched at all by an upward moving slime block
This is really interesting because I get issues like the lava examples in my singleplayer worlds. My breath meter will also flash when I walk on an edge near water. (1.14.60)
If this is what I think it is I'm also experiencing this issue. I have a portal with a 5x5 frame to enter the nether. The nether end of this portal is the same size. Going into the nether, these portals link properly but upon return to the overworld a new portal is created at opposite coordinates. I observed this on a superflat world.
Strange find: while making my own world to test this, I noticed that most of the issues went away after while of playing and being pushed and launched by moving blocks

I replicated this on beta 1.10.0.4 using /replaceitem to put a nautilus shell into the offhand slot of a Zombie, a Husk, a Drowned, a Zombie Villager and a Zombie Villager V2.
In my case the Drowned was the only one to hold offhand items without crashing the game.
After filling the anchor did you tap it, like a bed? If not, the respawn anchor won't be used as your respawn point.
