Still an issue in 1.16.0.66 beta. Attempting to change the colormap to the correct color will also cause normal forest grass to appear as roofed forest grass.
Please consider re-opening this. The game needs a proper fix. Simply changing the mipmapping level can cause other issues.
This has been an ongoing issue that I would really like to see addressed in the near future. It can get very annoying moving the cursor across the screen to get to items.
All Windows 10 versions 0.12.0 - 1.13 beta are impacted.

Still occurs on 1.2.0.11 (beta 4) on lower end/32 bit Android devices. Additionally it still occurs on 1.1.5 on most iOS devices and the devices just said.

As far as I can tell, this bug occurs on most iOS devices and 32 bit/low end Android devices (like the Moto G 1st Gen).
This issue seems to be fixed in the latest PE beta.

This issue seems to be related to numeric IDs of items in bedrock edition. For example, in 1.16.200 the iron axe (what the item in the chest is supposed to be) has the ID 298. The problem is that the leather cap also has the ID 298 at the same time. Items currently have two numeric IDs, so the game gets confused and places a leather cap This is a direct result of changing the item IDs in 1.16.100, and is also a massive problem with UI, where using the item renderer to render one item can show two different items at the same time.
