Found Leather tunic with "Efficiency I"
Seed 9052485491962549889

Original description
My friends and I raided a woodland mansion and one of them actually found a leather helmet with efficiency 1 on it! That shouldn't happen.

This issue seems to be related to numeric IDs of items in bedrock edition. For example, in 1.16.200 the iron axe (what the item in the chest is supposed to be) has the ID 298. The problem is that the leather cap also has the ID 298 at the same time. Items currently have two numeric IDs, so the game gets confused and places a leather cap This is a direct result of changing the item IDs in 1.16.100, and is also a massive problem with UI, where using the item renderer to render one item can show two different items at the same time.
The structure file in question (behavior_packs/vanilla/structures/mansion/1x2_a6.nbt) properly defines the item as minecraft:iron_axe
, but there's some issue with the NBT structure loader that is mutating the item ID. This is the only structure in the game that contains a chest with an item with predefined enchantments, so maybe it's something to do with that
Apparently a user on Reddit has found that the item is now a Nether Star. But like mentioned in the Reddit thread, it is unknown whether this is the default behavior as caused by MCPE-109048, or if the appearance of a Nether Star was caused by an add-on.

Bug: Efficiency 1 Enchanted Golden Apple
Just tried using the exact same thing to make a banner pattern, and it doesn't work. Absolutely FANTASTIC job, Mojang. Asking for an enchanted apple for something, getting one, and then saying "No, thats not the RIGHT enchanted apple."
