this happens when eating food too since the new update, when you eat while taking damage (most likely fire damage) and switching to a tool in your hotbar, your item gets deleted and replaced with a duplicate of your food source. see: MCPE-177351 i lost my op sword twice in a day and caught both times on video in that post.
However, when trying to reproduce it in a seperate world, i cannot seem to get the bug working.
