I experienced this as well. I determined that it was similar to bug MC-62958 (https://bugs.mojang.com/browse/MC-62958) and turned on VBOs in the visual settings, which seems to have resolved the issue for now. (I would have updated my Catalyst drivers for their listed solution but it produced a BSOD at each attempt).
