And btw I also found something that further confirms my theory
If the item is on top of a stair, and you throw the pearl on the air part of the stair, the pearl has enough space to land lower so you can pick up the item

Blocks which are related to obsidian are #dragon_immune (AKA: Crying obsidian + The respawn anchor)
So....
The rule that "It has to generate in the main end island" doesn't apply to those 2
So why shouldn't the ender chest be included if it is made of obsidian ?
+ All end city blocks (AKA : all the purpurs, chests, brewing stands, purple stained glass pane, banners, and end rods ) shouldn't be included in #Dragon_immune blocks
Because they have no relation/materials to/from The Main End Island
Except for a couple
Which are the end stone bricks and it's variants
Because it is made of end stone
And end stone is #Dragon_immune
So they do go under the same category as the respawn anchor

I just opened a flat pure vanilla world and tested
I teleported myself kind of close to the same coordinates, then put a command block to give me levitation, threw an item on the ground, the throw pearls on it and around it
And the result was:
Yes, it is a problem in vanilla
Once I go above the item by 3 blocks or more it works as intended
But if I am within roughly 2 blocks of it on the Y value then the bug does exist
