When logging in or starting up a LAN server, player appearance (Tool in hand, armor, heads, pumpkins,) Will not update for other players(2, 3, 4,) until player 1 has logged out and logged back in, and then it will update their appearance once to other players. Therefore leading to views from each player like breaking sand with a sword very fast or eating cobblestone.
Steps to Reproduce:
1. Start up LAN server.
2. Have other players join the LAN server.
3. Have player besides the host get a tool, sword, or block in hand.
4. Have them log out and relog.
5. Have player switch to a different item in hotbar from when they logged out.
6. Watch as their item on their hand does not change for any other players besides himself.

Or just a server, I got that bug too, because when someone had an axe, sapling, etc on hand, I saw they had an iron pickaxe on them.
I can confirm this error in our Let's Play on a vanilla 13w17a multiplayer server. When my friend logged in, I had a sword in hand and was wearing boots, and they could see that when they signed in. No matter what they put on, though, I was unable to see what they were wearing until they relogged. When each of us died, we appeared to be wearing our default skins again.
Thus, it seems to be an issue with the server not pushing to each of the clients when a player update happens with regards to items held or armor worn.
I do not know if this is related, but when a player is killed on a LAN game and they are out of loaded chunk range, then the other players are unable to see the first player until he or they relogs or dies.
