Nether portals don't generate or generate unlit

New as of 14w28a: The first portal you create in a world has a small chance of spawning you underneath the bedrock of the nether in the void, spawning you in a wall, and generating the portal unlit. After this, any newly created portal in the overworld automatically links to the original portal in the nether, no matter how far away.

Updated steps to reproduce:
1. Create a nether portal in the overworld and travel through it, preferably in creative due to the chance of spawning in the void, spawning in lava or suffocating to death. Remember your surroundings when you enter the nether.
2. Re-enter the overworld and create another portal any distance away from the original and travel through it. You should be in the exact same place as before.

The photo I attached from my phone was to replicate the issue I had run into in a freshly generated world, just in case something was broken from an earlier snapshot.
Importantly, it gives you all the details to replicate this bug yourself:
1. Create new creative world with seed: 529770237
This is the same as the seed: New Features
2. /tp -638 63 44 83 19
(Facing a really nice cliff over water, with a waterfall, lavafall, and stone pillar.)
3. Build a nether portal, light it, and travel
4. You were in Creative mode, right? If not, you are now dead. If so, you will not find a nether portal, at least I didn't digging around in the nether rack a lot.

For me the portal in the Nether spawned above a lava lake and I spawned inside the blocks I was supposed to be standing on, thus took suffocation damage and fell into the lava lake.

This bug does not seem to be fixed. I am running snapshot 14w26c and the description of this bug consistently happens whenever a new portal is generated. Once the portal is generated, usually traveling through it works fine. Although a second unlit portal may sometimes be generated.

Happened 3 times in a row to me.

Portal issues seen in a world with several existing portals in 14w29b:

1) Some existing portals no longer lit, both in the overworld and nether.
2) Some existing portals partially lit (one blot lit, all blocks but one lit), both in overworld and nether.
3) Going through portal landed in mid-air in the nether, falling on top of existing portal. No new portal.
4) Going through portal into nether and landing over lava (ouch). Returned to find new portal (unlit) about 6 meters from intended (also unlit) destination had been created on shore of lava ocean near where I fell.
5) Going through portal back into overworld and materializing in solid rock. Dug to surface to find second unlit portal about 6 meters from my intended destination (which was also unlit).
6) in a couple of cases, portals were entirely lit and functioned normally.

In all cases, making sure the portals on BOTH ends were completely lit by destroying any portal blocks in creative and re-lighting resulted in the portals working as expected. This suggests to me that portal coordinates were corrupted but that re-lighting the portal stored the coordinates correctly.

This was already fixed in the most recent snapshots

Was this the cause of Etho's portal teleporting him thousands of blocks away in one of his recent LP episodes?

Probably not. That portal worked but multiplied by 64 instead of 8. And he emerged from the lit portal, not elsewhere below or above it.
