As I commented, these are all in one post because they are all one issue. This issue is old as dirt but it's not receiving any attention from Mojang because it's been effectively stomped out in every ticket made. For some reason mods have decided that older versions of this ticket are invalid because they think that clouds are just terrible and broken by design.
I am putting these in one bug report because they are all the same problem. The code for clouds is fundamentally poor design and quality compared with the rest of the game. Reworking this code would likely solve all issues at once.

Happened 3 times in a row to me.
I can confirm that this is happening on non modded 1.7.9, 1.7.10 and 14w29b with 3gb dedicated to the Minecraft instance.
This is on Windows 8.1 professional JRE7.
Yes I know, and it's unfortunate this never got fixed while it was relevant. I suppose resource packs, display names and lore can all be used to the same effect though.

I have a video I can upload of this occuring.
The Hud, my hand and any contextual menus are still fine.
So far this has been triggered by TNT detonating, water being placed, and occasionally just flying around in creative. Resetting the texture pack and dinking with video settings do not cause the screen to come back to life. Only placing a torch seems to, so this appears to be a bug in the lighting engine.
I do have night vision applied and for an extended period of time, I'm not sure if that is related.
They display correctly from above, just not underneath.

Only while in hand. Has to due with the player entity not being marked as translucent.

Not occurring for me.
The glass block is not transparent and does not accept semi-transparent pixels. This makes glass appear ugly. Voting this up to get glass changed to a transparent block.

The problem doesn't occur when using the give command, the items stack over eachother when:
A. You have 388 in your inventory, then pick up 388:1 off the ground, or
B. You have 388 in your inventory, and shift click 388:1 out of a chest.
or vice-versa.
Affects 1.9. 😞
