If a map is zoomed in, the center of the zoomed in map lies always in in the south east corner of the previous map.
Since the center of the 1:4 map can´t be chosen freely on creation, it is only possible to create 1:1 maps for 1 out of 16 chunks.
How to recreate.
1) Get an empty map
2) Activate map with right click
3) Zoom in using shears

I think this bug explains my experience too: I made a map in my town, "zoomed in" twice using shears and got a completely empty map, obviously not showing the location where I was.
To be honest, I think the new recipe with the shears is a bad idea anyway. Even when this bug is fixed and it somehow becomes possible to create 1:1 maps of every area, it's still very counter-intuitive to have different recipes for zooming in and out, uselessly more complicated than the old recipe, and will require using the F3 screen and calculating to figure out where you have to stand, then use the map, then cut it with shears, and THEN finally see if it's the location you wanted.
My personal preference is clearly a revert to the old recipe.
Confirmed for 15w40b. This bug makes it impossible to map any sort of area in the game. It always seems to zoom into the bottom right hand corner as well, regardless of where the player creates the original 1:4 map.
Agree totally, That's a bad idea, old system was better. Now I can't even create normal map for myself... Center always go somewhere else... ("And sometimes map goes blank...", well player cursor just not showing till you closer to map zone) Let us create maps from 1:1 level.

It appears this is fixed thusly:
maps are now created at maximum zoom (like before the snapshots)
zoom-in-with-shears recipe removed (like before the snapshots)
For me in 15w35b, the maps zoom in on the lower right / southeast / +X+Z corner.
Perhaps explains MC-86753, since the player is not in the expected area, making the map not update?
