I have a 7x7 portal in the nether with a small closed area without light. The other portal (overworld) is in a mushroom biome and also closed off with dirt so no mobs could get it.
Sometimes 2 creepers and 1 skeleton randomly spawn in the other side of the portal (in the nether) when they couldn't possibly go though it. This has happened to me twice already.
It's hard to recreate and doesn't happen often.

Can confirm this is happening in 1.16.40 too. We’re having a problem with players on my server going through the Nether portal only have it blown up from a creeper. Players are stating this happens every day. The portal is surrounded by an enclosed structure on both the Overworld and Nether side. It’s spammed with torches and flow stone yet they’re still spawning in the portal.

Moved my overworld portal from Lighted basement to a Lighted upstair room and creeper spawn stoped for like a few days(irl) but this morning i went to nether and boom again.

Can anyone affected provide precise steps to reproduce this issue, a world save, or seed + coordinates where it occurs?
I have had this issue, too. I have completely lit up the area around my portal and even tried breaking it when I don't need it and re-lighting it later, but mobs still appear on the other side. My game doesn't load the fastest (MCPE), so the mobs are free to attack me for a few seconds. It happens very sparingly with no seeming pattern. I'm not sure how I would replicate it because of this.
It's a full gold armor skeleton in Noah's world downloand, not a zombie. It is not persistent (I was able to despawn it by moving far enough away in the nether), so it seems to have been be naturally spawned.
The world is named as a "realm", so is it possible another player on the realm was being followed by the skeleton without knowing it, and forgot to shut the gate?
Thanks for checking it out! Yeah my bad, that's a skeleton. The world is named as a realm because it used to be on a realm, not anymore, and I was too lazy to change the world's name.
I was keep googling about the problem and actually found the cause and it's not due to a bug; it's cross-linking with other gates. I have a zombie-pigman farm at (866, 239, -1213) with huge multiple gates, which is far less than 1024 block-distance from (959, 71 -1061) where my overworld nether gate located. I thought 128 is the distance I should maintain between the gates to avoid cross-linking but the number is for Nether, not overworld... Sorry for my confusion! I will deactivate other gates and see if I can still see creepers in Nether (hopefully not!!)
By the way, while I was fixing the cross-linking issue, I noticed hostile mobs spawn on the top of gates at y=256 regardless of the lighting conditions. It's in my z-pigman farm. I searched the issues but only found resolved cases for Java edition. Should I open a ticket or it's a duplicate I just missed?
@Noah: I have a report on mobs spawning at Y = 256 at MCPE-112682.
Given that Noah’s problem turned out to have a different explanation, we still need a world save or steps to reproduce the issue reported here. I am setting this report back to Awaiting Response.

Would it be possible to get a copy of the world save where this occurs?
Is there another portal in the overworld where these monsters could be getting in? It is possible for more than one overworld portal to send to the same portal in the nether.
