you are using an outdated version of minecraft.the bug tracker only accepts the latest versions.

i have one on a survival world seed.
i think getting out the hardcore wolrd folder out of the saves folder then putting it back in will make it show up.
can you damage players?if so,(and also unable to break blocks) sounds like you are on adventure mode.

this happened way back in 1.17 pre1.It was fixed though.

happened to me while exploding a village lol.was confused and was about to report the bug but it exists already
You must be using the programmer's art texture pack.
This phenomenon is known as "basins" and the generation works as intended.
I am guessing it's one of those most duplicated outdated graphics bug report?
