I have had this happen to me in 14w30c. Very rarely in small rooms (low ceiling, narrow hall, etc) item will pop off frame and then just disappear never to be found again.

More of an indirect bug due to how terrain is generated. If you play a game with low seas, then sugarcane wont spawn near oceans or rivers. I finally found one piece of cane next to a lake, but that was after hours and hours of searching. Very hard to play Minecraft without sugarcane, which leads to books and enchanting!
