I have this same problem on my REALM. How do I fix it?
A while back this issue was resolved for me. Did not know it was still ongoing as I haven't experienced it for a while. As well as, my realm is permanently closed due to chat reporting.

This is most likely to avoid any kind of "creative way" to insult someone. I personally do not think the filter is accedentally doing this, i think it is to avoid bypasses to insult. If you want a name Trapezoid, maybe try misspelling it? like Tr4pezoid, or Trapizoid, or possibly, Tr4p1z01d? and for grasshopper, Gr4zzhopper, or Gr4zzh0pp3r. I think it is possible to bypass that filter with the name combos i listed here. If they arent, just keep in mind, this filter is meant to protect mean comments in names.
Please note I forgot to add this back when this was alive I think this was just bugged due to it being right after I left my realm and I went into a single player world and accidentally clicked this. If I remember correctly it resolved itself after I closed the game. since this it hasn't happened. Kinda similar to how some server's stuff can display when your in a single player world / realm that isn't on the world your on, for example Hypixel's AFK message can display in a single player world if you leave at the right point and join the world at the right point so I think that's what happened here. Anyway I added this as a bug report because it would happen even if I left the singleplayer world and rejoined it, relaunching the game fixed. If this happens to anybody that's the fix
