Phantoms have their whole body exposed to the sum, so it wouldn't make sense for just the helmet to protect them.
If you want them to survive in day, give them fire resistance.
I think this is a feature request, not a bug.
WAI. If you hover over something in creative or survival and click a number, it swaps the thing you are hovering over with the thing that is already in that slot. In the creative menu, this means that the thing in that slot is deleted.

This sounds like a technical issue, not a bug.
If you were playing on a server, this may have been caused by server lag. If you could reproduce the issue and include a video, that would be helpful.
This is probably still invalid as you were trying to play on a spigot server, not the minecraft one.
You can probably solve this by reinstalling minecraft and/or deleting all worlds, but this is not really a bug.
You might want to try googling your issue along along with any error messages that you received. You may also want to try reinstalling the game and restarting your computer. If you cannot find a solution and the problem still persists, you should add the log file for the game to the comment. In general, I don't think that this goes on the bug tracker without further research first.
The issue linked as a clone is actually a different issue relating to the border between nether forests and soul sand valleys.

yea, I'm afraid you are on a non-vanilla server, so unless you can reproduce off of the spigot server, this issue is going to end up invalid.
