Wow, I just found my own post in the "Browse" section!
The only way the Biome Dome can kill you is if you try too frequently.
By the way, the biome dome puzzle shouldn't kill you, it just teleports you to the beginning.
Oh, and the book can be a good help, too. (It gives you the answer)

Quit the world, turn on cheats, set the default gamemode to Creative, start the world, start flying and look around for the spawn area.
Once you find it, run the following command:
/setworldspawn
This is not a Minecraft bug, the map creator just used /spawnpoint
instead of /setworldspawn
, so new players will spawn at the default spawnpoint (the sky)
What is code connection?
Attach the entire world folder in a ZIP, not just the level.dat.
Check how it looks without Smooth Lighting enabled.

My assumption is that Minecraft believes the "connection lost" screen is something that only shows up in multiplayer, and not on the host. (You probably won't lose a nonexistent connection!) The host usually starts the world from Singleplayer. However, the "Illegal Position" uses Connection Lost.
