I can confirm this is happening to me as well. 15w47c on OS X.
Still occurring for me in 15w32b. It seems to be caused by poppies entering the Nether from my Iron Titan. The poppies never arrive so the chunks will unload as soon as you leave them and break the farm.
This is still an issue in 32a. Can confirm its related to descending into the portal with creative flying. Does not happen in survival mode at all for me.
Still a problem in 15w31c for me.
How does one make it log to an HTML file?
My kernel is the latest version available in Ubuntu 12.10, 3.5.0-41-generic i686. Are you using OpenJDK, Sun Java, or something else? Do you think upgrading to Ubuntu 13.04 would fix it? I like my LTS releases, but anything for Minecraft 🙂
EDIT: I am upgrading my Ubuntu to 12.10 to see if it works, then to 13.04 if that doesn't work.
EDIT 2: 12.10 is still not working. I'm not getting into the game at all, even after a great delay. Please upvote for fix!
EDIT 3: After a lot of errors, 13.04 works! YES!
My Ubuntu 12.04 LTS box with Sun Java 7 will not launch 13w39a either. Its very new, just built it this April. It has Intel Graphics HD4000, which has served me well so far, the MacBook Air I tested on has the HD5000 but that shouldnt make much of a difference, they both support OpenGL 2.1. Please vote for this issue, its impossible to play on an 11" screen!
So how would I set up the Log4j thing?
Logging works fine on a Mid-13 MacBook Air with a fresh server installation. I can't figure out how to make it log to an html file though--supposedly this was added?

I was on OpenJDK 7, but it kept crashing both the client and the server randomly. I don't know what's worse, random crashes or weird lag.

oddly enough after a game restart i was not able to reproduce. i was in a different world then when i first spotted the bug though
