Maybe try deleting Minecraft off your computer and downloading it again. This shouldn't delete your worlds, texture packs, or settings as that data should be kept in your .minecraft folder.
Could you send a screenshot maybe?
No, it hasn't happened since either.
Perhaps it was a client issue or visual glitch?
Also, did the bugged out icon appear on the screenshot?
The recipe book is green with my texture pack; the issue was an unaligned/bugged out icon (I added the screenshot, it may not have shown it though)
