Only the item dropped when I punched it, and after finding it, I managed to reproduce it a few times with only the item dropping. The spot was somewhere in the lower right corner of the item in the item frame.

They don't seem to for me. Though, mobs do sometimes go into walls and suffocate, so that might be why. Foals are the only mob that do this consistently.

I'm pretty sure that bug was from a while ago, and I think it works with any world name.
