One Sculk Catalyst may contain more than one signals (controled by 'cursors' NBT) and spread to different places. If watching the subtitles, this issue is as well able to be reproduced.
I think the reason is that there's as well playing the same voice in any other places else and it's too far to hear but it yet occupies the subtitle sequence, resulting to the subtitle only shown for milliseconds.
According to my observation, this issue doesn't occur in a flat world without generating structures, meaning there's no other working Trial Spawners we cannot see. Then reproducing this, the issue won't happen.
I played the world again in 1.20.4 for several times and unfortunately I was failed to reproduce the bug again. Not very professional but I think it has sth to do with the amount of Unicode characters. Sorry!
1.20.1. In fact I think this issue can be solved by operating a resource pack in local. I've uploaded a screenshot of it; yet the official hasn't provide this source string (death.fell.accident.water) on Crowdin.
And I just checked the "en_us.json{}" in 1.20.1.jar and tried to search "death.fell.accident.water," but no results.
As for the Banners created before I think they have to be soon abandoned since just like several classic Redstone machines only available in earlier versions. What players could do in newer versions is only have to create new one of sth to replace the earlier one, having no power against the developers' mistakes. So in my opinion if Bedrock developers gonna fixing this, changing Banners only and remaining the Banner Shield's layout perhaps is the better choice.

Sounds a sort of unreasonable Trial Spawners has to be bound with that game rule. Appreciations for you all finding out what goes wrong anyway.
