Occurs in 1.8.8 while boating. Since they swim painfully slowly (thanks to MC-48616), it's easy to leave them far behind. If I don't enter water shallow enough for them to tp into before their chunk unloads, they unload with it. Crossing any decent-sized body of water requires going aaaaaaall the way around the edges and skimming the coast at regular intervals.
Still present 1.8.8.
Portals have been acting a bit inconsistently for me throughout the 1.8.x updates. I suspect there's something odd going on in the way it looks for available portals, because from time to time valid portals are simply ignored. Three examples:
I've had the same problem as the Marty--make a new portal, travel through it, then come back to the overworld into a new portal generated some distance from the starting one (despite it clearly being close enough to link to the first time through).
Once I entered a portal, somehow skipped past the nearby portals, and found myself deposited at a portal a long distance away.
On a couple of occasions, I've been traveling my usual route through old portals, and had it suddenly build a new portal in between the old ones. On destruction of that new portal, the old ones once again 'linked' together and functioned as they always had...well, until the bug showed up again, anyway.
I've had this happen to both horses and villagers. I keep the walls set far back from the portals now to prevent them landing inside those, but that doesn't help with glitching into the obsidian of the portal frame itself.
It seems to happen more often when transporting multiple through at once (pushing each other into the blocks?) and/or when the portal they enter through is wider than the one they arrive at. Trying those together might increase the odds of reproducing it.
@BoxFigs The server is 1.8.6, so it is after the "fixed" version (though not as current as I thought). But I just checked it out in 1.8.7 single player to be sure, and it still occurred.
Still having issues getting them to follow in 1.8.7. Please reopen.
@Torabi: Sorry, I know you're just trying to help. I'm just frustrated because they keep closing these reports claiming that they are duplicates of that bug. Like they just did this with this one.
Definitely chunk loading related. I have always had it occur when reentering the area where my pasture and stables are located. I pretty much always do so using a Nether portal; I'm afraid I can't remember if it's ever happened when I just walked there. The interesting part, though, comes from my stable area having been built on the intersection of four chunks. Whenever this bug happens, it does so within one chunk and (usually) leaves the other three alone. You can see where the edges of the involved chunk are by where the horses are free and wandering and where they're still tied up. I have dozens of horses tied to regularly spaced hitching posts, so the line of demarcation is quite obvious. I haven't noticed any particular pattern regarding which chunk it shows up in. It's happened in every one at least a couple times.
@Torabi: No, this is a different bug. Maybe they're both related to the same section of coding, but the symptoms are completely different. I say that as someone who has had both bugs multiple times. Actually, I've had both happening in different areas of my stable complex at once. I'm just lucky that way.
This is definitely NOT the same bug as MC-14826. I've had both. The invisible-but-still-attached one has popped up a couple of times, but I get this one all the time. The leads are lying on the ground and the horses are wandering far, far away from their hitching posts. This is quite a bit different than having a horse appear to be free, but be unable to ride it more than 10 blocks away from the hitching post. Trust me, when you've had both you can tell the difference.

I just had this happen in 1.6.4. I left a horse in the Nether, and when I returned, it was glitched partway into the wall it had been standing next to and was taking damage.
