Cannot reproduce in 1.4.5. What mods is the server running?
And what is the red error message?

When you say you are unable to connect to the server until you close the client and relaunch, is there any error messages stating what the issue might be?
I can't seem to duplicate the effect to the extent shown in your screenshots at all.
I've tried various combinations of video settings, but the most I can see is an expected sight change of lighting of the left side of the player hand.
I've tried to reproduce this, but only saw 1 xp orb which I was able to collect, and I can't be certain that I didn't hit the slime at some point giving me credit.
I'll confirm that the behaviour is as reported, though I believe its working as intended.

This ticket is invalid as it relates to a modded or 3rd party client/server.
Any non-standard client/server build needs to be taken up with the appropriate team, not Mojang.
Any plugin issues need to be addressed to the plugin creator.

What version of the server are you running against?
"Resolved", with a resolution of "Works As Intended".
Which mods are you running there?
I've definitely experienced this issues, but in my experience is rare. For it to be happening every time is interesting, and may help resolve it.

My guess? Some other process is forcibly taking focus away from the game. This causes the game to automagically pop the menu up.
Is this any type of renamed item? I tried with my main renamed pickaxe and it was still renamed once removed from the enderchest.
