This issue self resolved.
Still happening in 1.6.1 Windows 10 and Xbox editions for sure.

Hi Annette, From the main window after logging in click on settings and on the left you should be able to see Storage. I attached an image to this ticket on what it should look like. If you had a cached files there would be an option called Cache or something there. This is also where you could delete worlds, resource packs, and templates (likely more to like behavior packs). The files should have the packs name, I have found it pretty straight forward. Hope that helps.

I believe I have done most of these steps but I have gone through them again just to be sure. My comments in red.
Reset the Realm world save first, to clear any worlds and packs that might be applied - Had not done before, I did it as part of this testing. (Side note the reset world spawn is different, I have an older seed I guess. lol )
Apply the latest version of the resource pack to the single player save that you have
You might want to clear the cached packs first (Go to Settings→Storage, then Select each Resource pack and delete the cached data) - Verified no cached packs, I did do this on the Xbox a while back and did not have on the windows 10 machine at all.
You can then download the latest version of the pack from the store - It was the most updated but I went ahead and deleted it and downloaded it again just to be sure.
Make sure you apply the pack to your specific world - Done, verified the pack in the local copy. Was done prior testing.
Now open your Realm settings and choose "Replace World" - and upload your world save with the pack applied.- Done, was done prior testing
I went through all the steps and when the realm loaded it was still default textures. I did all this on the Windows 10 copy of the world, but all steps were done before except the realm reset.

Also for testing, I made a brand new world, applied the texture pack (Chroma Hills) and replaced the realm with that one and got the default textures upon login.
Can confirm seeing the same behavior, appears to happen when there is a lot of ex at the same time. Can reproduce via a mob spawner farm. Kill a number of the mobs then watch as the tool appears to freak out. I am on a realm.
I am seeing the same thing with villager disappearing from my Iron farm. Realms, playing on Windows 10 1.2.8 version.
I have two pods and it appears only one of the pods has villagers disappearing and it appears that the pod is on a chunk border with the line bisecting the villager pod (Pod is 3x2) and the border runs through the middle of the pod on the long side.
I check on the pod every couple of days and it is random as to how many villager s disappear. The Pod on the other side of the farm does not appear to have anyone disappear but that pod is in the middle of a chunk.
Impacts 1.16.2.0 release also parity item with Java. It is not a huge problem but wanted to update the status.
