This is the case for mobs as well.
It seems the main reason the spine and ribcage are offcenter is because the spine’s front and back faces are 3 pixels wide (odd), while the wither’s main head is 8 pixels wide (even). So it has to be off center in order to aline with the pixel grid.
I see two possible solutions to this:
Adjust the spine’s model and texture to be an even number of pixels wide on the front and back faces.
Adjust the main head’s model and texture to be an odd number of pixels wide on the front and back faces.
If one of these solutions is done, the main head, spine, and ribcage will be centered.
This is a matter of their pathfinding, copper golems attempt to travel to the closest chests first. Since the left chest was slightly closer (a straight path instead of walking to the right), it deposited the item there as that chest was also a valid target.
AFAIK this is working as intended.

This is likely from endermen, as one of the blocks they can pick up and place down are clay blocks. The reason there are so many clay blocks is because there is probably a lush cave nearby, which generates tons of clay. I’ve noticed this myself and it is normal, just endermen having fun placing blocks.

This is intended behavior, it was added as something for user accessibility.
That is the player locator bar, and new feature added in 1.21.90. It only appears in multiplayer worlds, and be disabled using the “locatorBar” gamerule.
