Confirmed for 1.14.2, seems to occur when teleported to unloaded chunks. Ie, max range of 10k blocks.

I read through and tried to copy the few tutorials of how this is implemented in minecraft without the use of code connect, and seem to be unable to replecate it.

I've been having this issue as well, I set up my own web socket running on my machine, and tried to connect directly using the /connect command. Got an error saying "Could not connect to (what ever it was)"
This also happens if the zombie pigmen kill the mob
i did a bit of testing and can confirm this is the same issue.
hope this gets fixed, it made me catch on fire
UPDATE: turns out this is true for any non-full height block. it happens even if the surface of the lava is below the top of the blocks collision
i deleted some comments i made because i found them to be incorrect. turns out i just suck at redstone and hadn't run into this mechanic yet

I figured out the problem
if i delete blocks under the slab it works fine aslong as the slab is on top half of full block

accually it is a bug i have 0.7.0 and no names on your head i'll post a new bug report
well try uninstalling MCPE and reinstalling it after about 10 minites to an hour and it will suposidly fix it
yeah you can close this issue, there was just some extremely weird lag happening that made just the hoe not get repaired until i left the end (i was using an enderman farm.)
i would have deleted this report if that was an option
