I can also reproduce using Minecraft 23w51b. Because the exit end portal generates at the floor of the world, the end portal blocks are placed outside of the world bounds, making leaving the dimension without dying impossible.
May relate to MC-93185 in some aspect as it makes overworld return impossible. A workaround could be to always generate a chunk of end stone or such at (0,y,0) in the End such that even if the island fails to generate entirely for some reason, there's still space for the portal to form.

This occurs because the ammount of endstone the main island is made of is randomly selected and unrestricted.
Since this got triaged I'd like to say I'd hope Mojang resolves this as WAI or a low cap of like 10k at most is introduced. The related bug MC-93185 does need fixing tho.
These low endstone end seeds are truly stunning and rare sights that I would hate to see removed.
