When you combine any two combinable things in an anvil, the end product disappears as soon as you take it out. I tried two damaged items, items and books, and they always disappeared.

It happens in Creative and Survival alike. More info: if you shift-click the output, you do properly receive the item. It's only when you regular-click it that it disappears.
Renaming an item also causes it to disappear when trying to take it out the anvil.
Works fine using SHIFT-Click to remove the renamed or combined item.
Happens in survival as well.
