So, I had an after-thought here. We have many people that would desperately like this to be fixed, but the devs are asking for more information. Lots of complaining, but only a few players have provided extended detail. So after @Joshua De Ruieter's comment, it got me thinking, let's meet in the middle and get as much information as possible, so the devs can either notice a pattern, or re-create. I thought of some sample questions in hopes that this will help give direction for future reports. A lot of what is listed here are from previous notations.
What device are you on?
Which version?
Are you on Wifi or Ethernet connection?
What is the realm name?
Name of Minecraft account? (if you feel comfortable)
Are you grounded or mid-air?
What item were you holding at the time? (bow, sword, etc?)
What was in your inventory?
What armor were you wearing? (although it seems like that doesn't matter, better to be safe than sorry)
Were any potion effects active during the time?
Can you record it on video by chance?
So far, here is the compiled information I was able to gather from this topic about this issue.
This bug has existed since the first report on version 1.0.0 all the way to the current version 1.2.16.3.
This bug happens on both realms and survival.
Sometimes all armor breaks, sometimes only some armor breaks.
This includes iron gear.
This includes diamond gear.
This includes diamond gear with varying enchants. (See below)
Fire Protection 4
Protection 4
Projectile Protection 4
Mending
Unbreaking 3
Feather Falling 4
Depth Strider 3
Thorns 3
Blast Protection 4
This bug happens with or without Fire Resistance potion effects active.
This bug can occur while holding a Diamond Sword (enchanted), Fishing Rod (not specified enchanted or not), or a Bow (enchanted).
If you are not insta-killed, you can be given the wither poison effect and taken very low (1-2 hearts).
If you are insta-killed, sometimes you respawn with an absorption effect active.
This happens on Windows 10, Xbox 1, iOS, and Android (varying versions from 1.0.0 - 1.2.16.3).
The death messages include the following:
"Killed by blaze."
"Player was slain by blaze."
"You Died."

I'd have to agree with @Karadine about the logs, it's a good suggestion @Mega_Spud. And as far as @Jonathan Kirkland 's thoughts, I'd also have to agree that he makes a fair point about it being misleading to be marked as fixed on this one in particular just because devs can't seem to reproduce. So instead of bashing the devs, let me ask this very specifically:

What info do you need to help reproduce this aside from a description of the issue?
Realm name?
Time of day?
Version?
Name of minecraft account?
etc.
@mega_spud - I see that this one is still unassigned and it's been exactly 3 months (from today) since the last comment on this one. I see that @@unknown ( @cwckr ) added a label of "bedrock" to this in March. Could you potentially see if this could be assigned to someone on your team or if anyone can provide an update please? I'd greatly appreciate it, thanks.
@Karadine - While I understand this is frustrating, would you agree that @Mega_Spud is asking specifics about your issue in order to actually get it fixed? I highly doubt he/she is asking just to cause further frustration. They have a system in place that players affected can just add a comment and mention them to ensure that it doesn't slip between the cracks and can be reopened if people are able to reproduce. I'm not saying your frustration isn't warranted, this is a long standing issue, but developers have metrics that force responsibility upon them to fix outstanding issues.I, too, have been frustrated. I don't mean to sound like I'm criticizing, but perhaps it's possible to try to see from @Mega_Spud 's perspective and meet him/her halfway.
As far as this issue goes, I'm still experiencing it on Windows 10 as well. It's not all the time, but it just seems to happen randomly in survival mode (I was on normal).

Hey, remember when a bug was reported almost daily, but with no communication whatsoever to the community that has purchased the game? I'm not asking for an instant fix, or a miracle here, but why aren't the devs or SOMEONE communicating with their customers on this one? The last reply from any mod or developer was November of last year =/
Devs, when is this going to get fixed? This issue has been unresolved for months now.

Zechariah, thanks for testing that, that's super helpful! I suggested to Nicholas to rush the end on creative and then change it to survival to kill the dragon and jump through the portal to help reproduce. Once again thanks a bunch!
I hope Mojang can get to the bottom of this. I know they are hard at work, but my friend has suffered from this same issue, and this is the second realm we've created. What's worse is that the only reason I bought this game was to play with my friends. I already owned the Java version.
I know this isn't necessarily at the highest priority, but I'd say this is one of the most important issues affecting players on realms it would appear. It's one thing for an item to not function correctly, but not being able to log in to the realm is downright game breaking.
A question for devs I would pose is, aside from the launcher automatically sending bug reports, and the thing I read earlier about followers and wifi, what might the status of this bug be? Do we have any leads on what might be the culprit here?
Sorry for the massive amount of questions Nicholas, my buddy is suffering from the same issue and you've provided a ton of detail. Have you been able to log on without wifi enabled? Hotspot perhaps?
Nicholas, how many friends and followers do you have on your Xbox account? I feel like I saw a topic earlier that was similar to this one, claiming that there might be a link between this issue and follower count.
I have experienced this same issue. Are you able to log into other realms unaffected by this issue? Also, what devices did you try specifically?
Is this one realms or single player?
My friend is experiencing the same issue. I am not sure how much this will help but it's on an Android device (Galaxy S8+) but also on XBOX and PC. He cannot join his own realm whatsoever. This affects multiplayer, not single player and he is able to connect to other realms just fine. I will check the wifi-disabled thing and let you know my findings.
This is a difficult one to prove, but amongst 6 - 7 players on my realm, all of which have fished collectively for hundreds of hours, and also explored almost every dungeon within our main map, I can confirm. We have not been able to locate a single tag. Could be bad luck I suppose, but it's comforting to know that someone else experiences this issue.
