Yes, it seems that the model in third person is rotated “upside down” than supposed to. You can tell by the bright part facing up on the spear.
I always wondered about this. That all tools have their bright side facing down, and their dark sides facing up, contradicting the direction of sunlight. Could this be unintentional? Maybe it's just to be consistent with the first person perspective, I'm not sure. It's definitely not consistent on the spear.
This bug came back in 24w36a and is still present in 1.21.11.

Sculk Sensor gets triggered by the sound
