I'm playing on Nintendo Switch, version 1.16.20, and experienced this issue. My first few attempts where with a beacon at around level 30, and no plan when building it out of gold blocks. When I didn't get the achievement, I tried breaking a few of the blocks of the pyramid, and reassembled again and powered it up. No luck.
After reading this bug report, I made a copy of the world, got a new nether star and gathered up all of the gold blocks needed, and rebuilt the pyramid. Completing each layer from the bottom up, and finally placed the beacon on top and watched the beacon light up. Without me needing to activate anything the achievement then ticked in.
Note, that since writing this report I updated the wiki based upon the in-game numbers.
And my changes got reverted, stating that 7 attack damage does indeed do 8 HP. Why/how ImakerB can see that is beyond my knowledge.
This also affects 1.7.0 when standing near water whilst sneaking, and getting hit by a skeleton knocking you into the water.
I did try searching for similar issue, but failed finding any resembling it. After talking on the official Discord server MCPE-33209 was brought to my attention regarding how opening the menu would stop sneaking/crouching.
Do you think this issue is then a duplicate of MCPE-33209, and as such is fixed in the 1.7.0.9 beta? Will this be officially released in the 1.6.x series, or not until we see the 1.7 release?
As indicated in MCPE-19503, it also affects the combination of iPad 2 and the Apple magic keyboard. In addition it also affects the combination of iPad 2 and a general bluetooth keyboard app running on a Mac Mini (Pre-2011).
This also applies for MCPE 1.0.1
Here are some more details to narrow down the bug:
It is player related! Friends of mine has been playing on the realm after I was shut out, and when I changed to another gamerTag I was allowed back on. So the bug is connected to a given gamerTag
Not sure if it is Blaze's or Wither Skeletons triggering it, as I've fought both (and died). Most of the times surviving, and able to play again. My final death was within a Nether Fortress
Stuff I tried to remedy the situation:
Restart MCPE, both just a plain restart, or by wiping it from memory (using the multitask handling of iOS)
Restart my iPad before restarting MCPE
Re-installing iPad without installing anything extra, not even the backup of my worlds
Re-installing iPad including my worlds
None of these actually helped allowing me back on the realm which crashed. All allowed me gameplay in different worlds, but not on the realm. The only thing allowing me back on the realm, was to create another gamerTag and be re-invited to the realm with that new gamerTag.
Do note that I'd already died some times in the Nether, before it finally crashed and never let me back in. So this bug is triggered by a death/crash in the Nether, but that is not the main issue, as I'm living (or dying? 🙂 ) proof that you can die some times in the Nether without being locked out.
Just a thought: Could it be that when dying in the Nether, the game tries to teleport you to your home location coords, but neglects that you are in the Nether? So what is actually happening is that it tries to put the Player at a supposedly safe place, but it could be all Netherrack (or Lava), and therefore it crashes?
I'm not sure if I can provide any diagnostic, but this does also happen on iPad 2, iOS 9.3.5, MCPE 1.0.0. I died in the Nether, and can't reconnect. It gets through all loading messages, and then crashes when it should display me in the Nether.
I've experienced the same using an iPad 2, running iOS 9.3.5, and MCPE 1.0.0. After I died in the Nether, I can't reconnect to the realm. I'm not the owner, but still an admin interface to reset a players location would be greatly welcomed!
So making a Grass Path through somewhere, and wanting to fence it off means you have to have a Grass block under the fence gate? That doesn't look nice!
I just tested this in iOS version 1.0.0. Given that the sun rises in the west, and goes down in the east the following still applies:
The stem of the pumpkin points to the North-East
The sunflower faces east
The bright spot of Glowstone block is in the North-East corner of the top face
The "L" on the cobblestone is also in the North-East corner
That is the good part, but still the following applies:
Rotating using /tp ~ ~ ~ 0, make the Player face due West (not due South)
Positive changes in x coordinate move the Player towards the South (not East)
Positive changes in z coordinate move the Player towards the West (not North)
So the bug is still in action, or the coordinate system is somewhat mirrored or warped.
Cannot reproduce, as I've gotten the achievement already. It should be easy to reproduce though with a fresh user, and if not please ask for clarification on procedure.

The same does happen on iPad 2, iOS 9.3.5, with MCPE 0.16.1. Loads of bubbles, but fishing appears to work (maybe with the fishes having more bubbles and going a somewhat stranger path than before).
Or maybe it's a bug in MCPC, as well as a bug in MCPE? To me, it's not natural that two disconnected water streams should affect each other visually as these does, so even though there is much to be said about simplified physics in MC, this sure feels like a visual bug.
I tried the tip from Kyle Miller, by going to Options > Sign Out, and then Sign in, and it did allow me to start getting the achievements. So this does reset something re-enabling the possibility to get the achievements.
I've attached four images displaying that the bug still persist on iPad 2 running iOS 0.3.4, with MCPE 0.15.6. The four images is talking from a world with the seed "mimi". IMG_1052 is the sunset with player facing West, IMG_1054 is facing North with map, and IMG_1055 is facing North without map (but with sign), and IMG_1052 is sunrise facing East.
Note the placement of the pumpkin and the glowstone which in the default texture pack helps determining the direction. Stem of pumpkin going towards North-East. And the lighter pixel (and area) in the South-West corner of glowstone, or brown edges in the North-East corner of glowstone.
I tested this in a world on the seed "kph", and around location -2304, 44, -1311 in creative mode. Also tried a little in a closeby river in survival mode, and also there I only got corals and seagrass.
