The command he was using was
/give @p minecraft:lever 1 0 {CanPlaceOn:["minecraft:wool 2"]}
Cannot reproduce in 1.11.2
The trick that i suggested before is really hard to pull off normally, so I find it unlikely that it can be done without really trying
Can confirm in 1.11.2
Confirmed in 1.11.2, in my case I found it to be connected to the "Can't keep up! Did the system time change, or is the server overloaded? Running 27608ms behind, skipping 552 tick(s)" while trying to use too high settings for my PC
Minecraft 1.11.2 HAS been released, but it doesn't let me update the versions affected

It's very random, but I found it more likely to happen when immediately after catching something you spam the right click
It might help to reduce the Max Framerate, to replicate as closely as possible what happened for my old computer
Windows 10 Home x64, Java 8, Intel Core 2 Quad CPU @ 2.50GHz
Minecraft Java basically on the lowest settings except render distance (7)
In my opinion, you can flag this a a dupe of MC-46212 but it shouldn't have been closed
Not a dupe, MC-1794 is about blocks and ALL the blocks, while this is an issue only with the fishing rod model/texture
I think that I might have found the cause:
When you catch something and then you double-click mouse 2 you might accidentally reel in the item that was already coming to you, and that uses 4 durability
Actually i still found it in 1.11
I have a screenshot if you need it
A Mod should make a "New feature" report based on this
Can't reproduce using @unknown 's solution in 16w42a
-Added the resource pack used
I used the1 1.9.4 version because there isn't a new one NOT RELATED TO THE BUG
The title screen is in the same location for 1.10.2 and 1.9.4 resource packs: ResourcePackName.zip\assets\minecraft\textures\gui\title\mojang.png so it has no reason not to work
Confirmed for 17w06a
