I don't think this is driver related, unless Intel and NVIDIA share code.

I'm using Minecraft v1.16.201 and here's the CPU specs:
Windows 10 Home 64bit
Intel Core i5 @ 3.3GHz
8Gigs of RAM
Intel HD graphics (1.6Gb of memory allocated for it)

I can confirm that turning off "Fancy leaves" worked for me as a workaround.
I'm sure I was annoying everyone on the realm as I tried to open this chest a few times in a row, crashing the realm each time. Sorry guys! Breaking the chests did fix it, thankfully!
@GoldenHelmet 1.13.1 is out for some platforms. I have it on Win 10.
I can confirm this is happening. It doesn't seem to happen with every portal though, which is very weird. I've uploaded a short video clip of it happening.
This happens on a Realm I play on. We have found that once discovered, if everyone logs off of the Realm for about 5 minutes, it fixes itself. It happens in the Nether and the Overworld, not sure if it's happened in the End for us.
Happened to me as well. I mined into a zombie dungeon on a realm, and killed the zombies, torched up the spawner, and they kept spawning. I went back to check my grinders' spawners, and the on/off lighting swtiches don't stop them from spawning either.
This just happened to me on a realm, and I'm using Windows 10, Minecraft version 1.10.1. It transported me to the nether at the same coordinates the overworld portal was that I entered. I also was eating. I'm going to try and recreate this when I get back to the portal in the overworld.
Edit: I was able to duplicate it and it seemed that i had to finish eating at the moment the portal transported me. The first time it happened, there was no nether portal generated in the nether when I arrived. I placed a quartz block with a torch on it and tunneled to the nearest portal on the realm. The second time it happened, a portal did generate! Of course it doesn't take you back to where you came from, so I destroyed it. The third time, a portal did not generate. This is the one I captured on video. Each time I entered the nether at the same place.

Still an issue with the final 1.1 release.
This isn't limited to realms. There is a bug report for this already, I just can't remember the link/number. It happens in survival and creative, extremely frustrating to say the least!
I downloaded the world to my device to see if it still had issues. Obviously it didn't disconnect me, but every couple of minutes the game froze. I imagine when this happens on the Realm it doesn't just freeze, it disconnects everyone.
Also, since I have the world file locally now, the seed is -90145806 if that helps at all.
This happens to me with the Natural texture pack on Android. It is a bit strange that only the wool shows up.
I have a friend who's also unable to join my Realm. The device used is an iPad and it shows up as full, 10/10 players when there's only one (me) on the Realm.

I can confirm this has happened to me, and is so frequent it makes gameplay quite annoying.
Attached are some screenshots of this. The items don't go missing, nor do I get "extra" items as it would seem, it's some sort of display bug I guess.
I've had this happen when placing any block where I'm apparently "too close" to place it. It looks like the block is placed for a split second then vanishes, and is no longer in your inventory.
This was on MCPE 1.0.4
Yes, I concur with Baxter that when zooming out, all previously explored area returns as unexplored on the map. It's only mildly frustrating compared to the empty map though...
This happens to some players (myself included) on my Realm. It seems to have to do with time of day for me, oddly enough. Around 1-3pm EST, when I first come on to connect and play on my Realm, I am liable to encounter major lag in breaking/placing blocks, opening chests, and other actions. Usually this is a sure sign that my client is about to crash. This happens on none of my local worlds, only on Realms. I was looking through the Minecraft folder on my android device and could find nothing that resembles a crash report.
I am running MCPE 1.0.2 on Android Marshmallow.
