Some quick math:

portal near spawn has coord (-9, -12)

portal in the nether (3, -17) which translates to (24, -136)

the distance between these is greater than 128(~128.3), so a new portal is created.

Maybe mark this as WAI.

Modded game is not supported here. However, I can reproduce in vanilla using the following setup:

  1. Create a new superflat world with seed 0, choose Overworld preset and change its content to minecraft:bedrock,63*minecraft:deepslate,63*minecraft:stone;minecraft:plains

  2. run /tp @s 92 62 124

I'm not sure if this is the intended behavior though.
